Our Services

We visit patients anywhere the patient is, including their homes, assisted living facilities, and/or through telemedicine. Our qualified and compassionate team of healthcare professionals provides exceptional home-based health care services to patients. Our providers can manage medication reconciliation, Home Health referrals, transitional care management, disease management, medical equipment orders, therapy evaluations, podiatry care, and other specialty referrals.

Transitional Care

Our Transitional Care Management (TCM) team assists with the hand-off period between the inpatient and community setting. After a hospitalization or other inpatient facility stay (e.g., in a skilled nursing facility), the patient may be dealing with a medical crisis, new diagnosis, or change in medication therapy. With regular visits from a Nurse Practitioner, our TCM services enhance quality of care, improve patient outcomes, reduce readmission rates, and boosts patient satisfaction. In addition, there is no co-pays or fees, so the it does not cost patients anything.

 

Transitional Care Management services involve a transition of care from one of the following hospital settings:

  • Inpatient acute care hospital

  • Inpatient psychiatric hospital

  • Long-term care hospital

  • Skilled nursing facility

  • Inpatient rehabilitation

  • Hospital outpatient observation

  • Partial hospitalization

Medical Checkup

Chronic Care Mangement

Chronic Care Management (CCM) encompasses any care provided by healthcare professionals to patients with chronic diseases and health conditions. The primary goal is to help patients to achieve a better quality of life through ongoing care and management of their conditions.

Chronic Care Management services provide benefits to the patients such as:

​

  • Contact from a nurse practitioner and clinical sta­ff between monthly visits to assess the patient’s health care needs.

  • Coordination of care with the patient’s other health specialists

  • Detailed reviewing and updating of the patient’s medication list

  • Reminders about preventive services and education

  • Access to care 24 hours a day, 7 days a week

Medical Consultation

Palliative Care

Palliative Care Management (PCM) services those patients who aren’t quite ready for hospice, but need more care and coordination. Palliative Care is focused on providing relief from the symptoms and stress of a serious illness. The goal is to improve quality of life for both the patient and the family, and it can be provided along with curative treatment
